How to Register
Registration is currently closed. You may register on-site in Room 101AB of the Ocean Center during the following hours:
Sunday, April 28 | 8:00am – 2:30pm, 4:30pm – 7:00pm
Monday, April 29 | 7;30am – 1:00pm
Tuesday, April 30 | 7:30am – 4:30pm
Wednesday, May 1 | 7:30am – 1:30pm
Registration Rates

- $375 By March 15, 2013
- $450 After March 15, 2013
- $200 Student Rate by March 15, 2013**
- $250 Student Rate after March 15, 2013**
**Letter from supervisor confirming student, fellow, or resident status required to receive student rate. Registration fee includes attendance at the banquet, evening receptions, program, access to the online program library, and commemorative t-shirt.
Exhibit Hall Guest Registration

- Registration Rate: $50
*May be purchased on-site only. Must be purchased in conjunction with a full registration.
Includes receptions, continental breakfasts, and breaks. The Monday night banquet is not included.
Refund and Cancellation Policy
A refund (minus processing fee of $50) will be given if written notice of cancellation is received on or prior to April 1, 2013. The USF Health reserves the right to cancel this program duet to unforeseen circumstances. In the event of such cancellations. In the event of such cancellation, the full registration fee will be refunded. USF Health will not be responsible for travel expenses incurred by participants in case of cancellation.
